Member NOTICE: Surchage Increase

Please be advised that effective March 1, 2013 there will be an increase in the surchage remitted to the Black Car Fund from 2% to 2.5%. Below is a sample notice that members can use, to send to their clients.

• Then Submit your claim online via NYS WCB: Online EC3 or you can print and mail the Employee Claim (C-3 Form) (You need to mail it to your nearest District Office )
